For unplanned hospitalization, your family can provide input into discharge arrangements.
When creating your discharge plan you should identify more than one choice of facilities and determine if your health insurance covers this stay. If there are no beds available in the SNF that was your first choice, the hospital discharge planner will transfer you to another qualified facility. Make sure the hospital staff knows about the places you have chosen.
