Meadowbrook Care’ Center’s residential and inpatient programs enable our residents and families to focus on living as fully as possible despite having a life-limiting illness. Palliative care brings this holistic model of patient care to those in the earlier course of a serious illness.

According to the National Hospice and Palliative Care Organization, each year nearly 1.4 million patients living with a life-limiting illness receive care from highly trained professionals in hospices throughout the nation.

Our hospice and palliative care program, combining the highest level of quality medical care, with emotional and spiritual support provided to their families whose loved one is facing a serious illness or end of life, provides pain management, symptom control, psychosocial support and spiritual care to residents and their families when a medical cure is not possible. These programs combine the highest level of quality medical care with the emotional and spiritual support that families need most when facing a serious illness or the end of life.

We invite you to learn more about hospice, palliative care by visiting us Meadowbrook Care Center, 320 W Merrick Rd, Freeport, NY 11520.  Or call (516) 377-8200.
